Counties Junior Representative trials registration
Hello everybody,
We will be having a registration evening on Friday 24 June 2011 - At CMHA
pavilion for Junior Representatives wishing to Trial.
Any person interested in trialing for Hatch / Collier / Yr 5 & 6 Girls or
boys, Must completed the Correct Paperwork.
* Attached Registration form
* Headshot Photograph of person wishing to trial.
* Copy of either Passport or Birth Certificate.
You can either, bring the above paperwork to the CMHA Pavilion on Friday 24
June 2011 between 4pm to 5.30pm. Where we will be taking photos and
accepting registrations
OR
You can post it to CMHA, PO Box 50, Pukekohe by Friday 24 June 2011 -
Lunchtime.
NO Registrations - No Trial.
Once Registrations have been collated, CMHA will be in contact with those
trialing to inform you of trial dates and Skill testing Dates.
